Toe pain is a common complaint with runners. Metatarsalgia is a word used to describe toe pain but isn't very specific.

Big toe pain can be caused by the sesamoid bones under the joint. Bunions also cause pain in the big toe, as can bony growths on the top of the toe joints.

Morton's neuroma is another common diagnosis causing pain between the 3rd and 4th metatarsals heads. The theory is a compressed nerve between the two bones which is made worse if there is a mechanical problem.
