Default Shoulder injury hasn't heal do I need surgery?

I posted here before like 2 months ago about my shoulder..supposedly I had strain on my shoulder that showed on MRI that i took.My doc told me to take physical therapy for 2 months and the pain was reducing a little bit but still had pain. Now I'm done with physical therapy and it been 3 weeks and the pain is even worse.I've been doing the excersices they told me to do at home but it's not working.I feel my shoulder is numb and my shoulder trapezius always hurt on the daily basis and it affecting me ...I try to run yesterday and I couldn't because after 10 min I started feeling a Very sharp pain in my trapezuis area.and also i cant even sleep because it aches at night i have to sleep on my good shoulder ...I'm going to the doc again tom.should I ask for another MRI ? Or do u think i need surgery because I feel like physical therapy is not helping . Sorry for the long post P.S I had this injury since august.

Has your neck been examined? Shoulder pain where some numbness is felt may be coming from a trapped nerve in the neck.
Ask your Doctor to look into this.
