    Default foot/ankle pain

2 years ago, i slipped and landed with my hip on top of my left foot, which had folded underneath me. i was at first treated for a sprained ankle, despite my foot being purple, swollen, immobile and stretched into the position a ballet dancer assumes when on pointes. subsequent x-ray, later in day, revealed a fracture of the 4th & 5th metatarsal, so I was put in a cast for 6 weeks.
I was off work for 4 months due to "soft tissue injury" and was signed off the sick as fit to work without any examination or tests. I have suffered pain in my ankle, instep (top and underneath of foot) and ball of my foot with stiffness and swelling ever since. Also the sole of my foot, under the instep always feels hot. I have tried physio (helpful in getting back mobility), orthotics from podiatrist and NSAIDs, to no avail. My Gp has refused 3 times to refer me to a consultant saying my last x-ray revealed no abnormalities and i should just carry on as normal, taking NSAIDs to relieve pain and wear an arch support (even tho' I told him this was very painful and NSAIDs inneffective).
Resumption of my normal activities of dancing, hillwalking and generally keeping fit aggravate the condition and the pain keeps me awake at night too, so I am fairly sedentary these days. Also I cannot wear heels or anything with an arch support as I am in agony. I still do my physio exercises, to keep my feet supple. Can anyone suggest what I can do next, please? Thanks in advance, Ruth
