    Hi
A rheumatologist deals with many conditions, mainly associated with joints, including rheumatoid arthritis.
Typically the rheumatologist works with other physicians, sometimes acting as a consultant to advise another physician about a specific diagnosis and treatment plan. In other situations, the rheumatologist acts as a a manager, relying upon the help of many skilled professionals including nurses, physical and occupational therapists, psychologists and social workers.

I don't think your insoles are likely to have contributed to your knee problems and if you wear them for a fallen arch (or overpronation) then it is possible that they have even prevented the problem from being even worse (especially given your MRI notes state 'medial malleolus').

There is always a reason behind a series of knee problems and if you don't get to the underlying problem (cause) then the problems will continue. Check this out, I think you will find it very valuable: http://egoscueportland.wordpress.com/2010/01/26/cartilage-damage/
